First, make sure that you have not entered HSA contributions twice.  Do  not include as personal contributions any amount shown with code W in box 12 of his W-2.

 

Make sure that you have completed the HSA section to establish his eligibility to contribute to an HSA.  He is only eligible to contribute for the months that he is eligible.  Does he have disqualifying coverage such as Medicare for any of the months?  For example, if he is covered by Medicare for the last 3 months of the year, his contribution limit is 9/12 of the annual limit.

 

Are both of you eligible individuals and one of you has family HDHP coverage?  If so, both of you are treated as having family HDHP coverage and your combined contributions are subject to the family contribution limit, although each of you is separately subject to the catchup limit.

 

If these don't address the issue, how much of the $4,836 is being shown as excess?
