Form T1135 is used to report foreign assets for Canadians.
Are you preparing a tax return in the US or are you in Canada?
If you are in the US and you do not have any assets in Canada, I suggest reviewing your input in the interview screens in TurboTax. You may have accidentally selected yes to having a foreign asset or bank balance that is generating this form.
