As you said, you can only deduct the premiums while you were self employed.
Also, if you are eligible for employer-subsidized insurance, that disqualifies claiming the Self Employed Health Insurance Deduction. For another point, if your insurance was through your employer, that is already pre-tax so it doesn't qualify for a tax deduction.
