All contributions made via payroll deduction, whether your money or free money from your employer, are considered “employer contributions” and are listed on your W-2 in box 12 with code W. (Basically, you agree to a salary reduction and the employer makes the contribution for you.)
Later in the program, you only list as your own contributions, money that you contributed separately out of pocket (after tax).
