PER IRS: you may still claim the child and dependent care credit when you're missing the provider's social security number or other taxpayer identification number by demonstrating due diligence in attempting to secure this information.
- Revisit the Deductions and Credit section.
- Click Update next to Child and Dependent Care Credit.
- continue to the screen Let's get some info about your care provider
- Select Other then Provider refused to provide The Tax ID number.
- Enter the care provider's information
