Pensions 2 & 3 would not be taxable as long as they were rolled over within the 60 day window. The 8915-F is set to automatically calculate your carryover. Note to your other comments about the program: The IRS has certain requirements for software facing the general public. A tax professional does not have the same system. You might like the desktop version of the software. It has the same walk through process but also has a Forms Mode that you can switch to and see errors, calculations, data source, and more. The downside is only you have your tax return so you need to keep a digital copy safe. If you want to switch to desktop, it is very easy. Switching back to online is not as easy.
