Yes, IRMAA is allowed as a medical deduction on Schedule A, which could come off against your adjusted gross income (AGI).  

Put the amount in Medicare D Premiums Deducted From Your Benefit. See the below screenshot.

These are the amounts your itemized deductions must be higher than to start making a difference in your taxes:

  • $12,000     Single or Married Filing Jointly
  • $18,000     Head of Household
  • $24,000     Married Filing Jointly or Qualifying Widow(er)
  • $+1,800     Extra for blind or over 65
