They are all definitely from the same place/interview. Here's the sequence of questions and my answers: 1) What type of HDHP did [spouse] have on 12/1/20? (Ans: None, though she's covered by mine). 2) Did you spend more than [xxx] on medical expenses? (Ans: No). 3) Before we ask about your Form 1095-A we need to ask about some other information. Did you or your spouse get unemployment compensation? (Ans: No). 4) Did you receive Form 1095-A for your health insurance plan? (No. She's covered by my insurance, for which I got a 1095-C). It's at this point that I'm told I have to enter a 1095-A before I can file or it will be rejected.
