1. Page 4 at the very top has an arrow where to its right is a space for TT to insert the number of additional boxes checked on page 1. It is the only personal data that this page can carry.
2. Page 4 covers both blindness and age boxes. The number there could range from 1-4
3. Page 4 is treated as part of the return by TT as it is both specifically numbered as such and if you direct TT to print "only the return to be filed" it is one of the pages that is in fact printed.
Therefore my inquiry to TT for clarification of this matter.
