In TurboTax, please do the following:

Child and Dependent Care Credit, click Start
- Did you pay for child or dependent care in 2016?, click Yes

Whose care did you pay for?, select your dependent

Who provided the care?, you will select A person, click Continue
- Let's get more info about your provider., you will be able to report his/her social security number
