The HSA bank will only send a 1099-SA if you made withdrawals in 2019. If you only made deposits, you will get a form 5498-SA, but probably not until May. If you made withdrawals and did not get a 1099-SA, contact the HSA bank directly; the form comes from the bank, not your employer.
