International student without US income, almost 5 years on US. Should I file a tax return form?

Im looking for help about tax purposes, I had been on IRS department, but they told me to call to International Department, which is impossible…
I arrived in US on January 25, 2011 as B2 (tourist), before my expired date I applied for change status visa, so like a tourist I had 340 days on 2011 calendar and 3 days on 2012 calendar.
As a student I had 362 days on 2012 calendar, 364 days on 2013 calendar, 364 days on calendar 2014, then 2015, my student visa will be expired on July 16, 2015 I’m confused because Im in US almost 5 years, but 1 calendar tax as tourist and 04 as student.
Also I have credit card without SSN information from Citibank and Macy’s; nonetheless some companies got my drive license # as SSN in their files (Comcast, real state, PECO) since Im leaving here and I have utilities and rent here, but some credit card company made the same, and Im using the credit card with wrong information, I didn’t know this, I just got this information few months ago, when the company called me and asked my SSN.
I don’t have income in US, my income comes from my country, and Im as my dad’s dependent for tax purposes, Im not making my country tax return since 2012 calendar.
I had filed W-8BEN Form 2013, 2014 on Citibank.
I know it’s confused, I really tried help on IRS Office, but they couldn’t help me.
So do I need to apply for Taxpayer Identification Numbers (TIN)?
Which Form I need to fill? Which steps to do it?
