Deductions & credits

Did the corporation pay for the insurance (either directly or by reimbursing you)?  If it did, that amount was required to be added to Box 1 of your W-2.  Was that done?

 

If yes, check that box AND when in the K-1 section check the box about medical insurance and answer all of the followup questions (but DON'T enter the cost of insurance in the K-1 section).

 

If the corporation did not pay for the insurance (and therefore did not add it to Box 1 of your W-2), DON'T check the self-employed box in the 1095-A section.  You don't qualify for it.
