It seems as though your cooperative has given you a 1098 or the equivalent of one that shows your portion of the mortgage interest.
If they did not specify the name of the mortgage lender, it would be appropriate to just list the name of your housing cooperative, or you can simply call the board and ask them for the name.

Enter Your Mortgage Lender- it could be the name of your housing cooperative, or you can simply call the board.
