In TurboTax Online, on the left-side menu, go to Tax Tools, then Tools, then select Delete a Form from the pop-up menu.
Scroll the list of forms for Form 5695 and click Delete on the right side.
For TurboTax Windows, click on Forms at the upper right. Type Form 5695 in the Open Form Search area.
When the form opens, choose Delete Form at the bottom of the page.

You may need to clear and start over if you can't determine the source of the info that keeps re-creating the form.
