Yes, you include any Medicare supplement and advantage premiums paid like on the federal return.
The Self-Employment Health Insurance deductions is included in the Wisconsin income (because they use the federal AGI and does not subtract.)
From the Wisconsin instructions:
"Self-employed taxpayers who deducted 100% of their medical care insurance on line 16, federal Schedule 1 (Form 1040) are not allowed an additional deduction."
