Are you saying that you did what I suggested as the workaround, that your 1099-R amount did not appear on line 4b on the form 1040, and that the amount that you entered as a personal contribution did not appear on line 12 of Schedule 1 (1040)? I have to assume that you did not do what I suggested.

The workaround will not stop you from filing electronically. The reason I asked you to make a note of this was in case in the future you didn't remember why you did not enter the qualified HSA funding distribution on line 10.

In your situation the qualified HSA funding distribution should not appear on line 19.
On your 1099-R, what was the distribution code on the form?
