State tax filing

If you are an unincorporated sole proprietorship, your business entity name is your personal name, unless you register a d/b/a ("doing business as") name with your state or county. (You may want to look into doing that for various reasons.)
(The IRS does not actually care under what name you operate the business. If you need to list something for Ohio, list your own name unless you get a d/b/a.)
