Clarification on instructions given with TurboTax Business for New York State Return

Multiple Questions
1. The $13 is attached for from CT-3-S?
2. I'm attaching a copy of my federal return even though the federal return was already submitted through Turbotax Business?
3. Form CT-400-MN is completely separate from Form CT-3-S? As the amount I owe also adds up to $13 (5+4+4).
4. When it says check for each amount for Form CT-400-MN, Do I need to write a check for $5, $4, and $4?

5. On a later instruction page it says I have elected to pay my balance through Direct Debit. Does that mean I don't need to send a check in?
6. Why is it telling me to pay using Form CT-200-V if everything is through Direct Debit?
