If you have already e-filed your Ohio tax return you would need to mail in the Ohio IT 40P payment voucher along with your check or money order, make payable to Ohio Treasurer of State.
The payment voucher should be included with your printed tax return. You can also print the voucher here http://www.tax.ohio.gov/portals/0/forms/ohio_individual/individual/2016/PIT_IT40P.pdf.
