If you live in a state that has an income tax and taxes were withheld from your checks, please contact your employer or HR department for the ID #.
If you live/work in a state that doesn't impose a state income tax, just leave the state abbreviation box blank, along with the rest of the state info boxes. You should be able to finish up with that.
