You can delete the form by doing the following:
- Log into your account, and then click on My Account on the top of the screen
- Then click on Tools and select Delete a Form
- Scroll through the list of forms. When you find Form 3554, click Delete next to it
- It will confirm that you want to delete the form, hit yes (or continue)
- The form will be deleted
