West Jefferson has a city income tax. It is not a member of RITA or CCA, so it does not have a code. Furthermore, you cannot prepare a W. Jeff city return in TurboTax.
Contact the city for forms. Or go to city hall, they'll do the return for you (take your W-2).

There is no "West Jefferson" School District. 4901 is "Jefferson".
