You would need to send the MO-1040-V payment voucher along with your check or money order. The payment voucher should be included in your printed tax return, you can also obtain the form here http://dor.mo.gov/forms/MO-1040V_2016.pdf.
Make your check or money order payable to the “Missouri
Department of Revenue.” Be sure to write your name, address, SSN, daytime telephone number, and
“2016 MO Income Tax” on your check or money order.
Your income tax return and payment are due no later than April 18, 2017.
