Q. Do I check the box that I lived out of state?

A. No. Being an out of state student is only a temporary absence from you home state. This is particularly true, in your case, where you have actually returned to you state of residence. 

 

Even if you need to check the box that "you earned money in another state", you say no to the "lived in" another state question.  

 

You would also answer no to the  earned money in another state question, if the school location state is "reciprocal" with your home state.
