this information is collected during the final steps before e-filing (I don't recall it being optional or what the other alternatives offered were); I see both the ID and issue date on the worksheet but it was not included in the PDF of the 760 for my records. If you're e-filing it will get handled during that process, if filing manually I suppose you can just write it on the form.
