The numbers don't matter, the mistake is using that worksheet in the first place.
The worksheet does not apply to return of excess contributions to a Roth IRA.
As I said at the beginning, you have somehow told TurboTax you made a Traditional IRA withdrawal, which, according to you, is not the case.
