First, a 1099-SA is not normally income, and there is no way to show withholding on it anyway.
Look for the "Schedule for Ohio Withholding" on your Ohio return. If you have the CD/download version of TurboTax, you can go into Forms mode and look for your Ohio forms.
If you have the Online product, you won't be able to see this worksheet until you pay for the product.
You can see this Ohio reject code here on page 19. Unfortunately, it doesn't tell you anything more than what you already know.
