It's not clear what you mean by "insurance/benefit surcharge" or "health insurance surcharge deduction".
Q. Where on the W2 is the insurance/benefit surcharge?
A. There is usually nothing about health insurance on the W-2 that affects your income tax filing.
Employers may put the cost of the employer paid portion of your health insurance in box 12 with the code DD. But the portion you paid is not usually shown on the W-2. Your employer may include "health insurance premiums deducted", in box 14 of the W-2, code V.
But, that is usually not something you can deduct. Your medical insurance premiums are usually paid with "pre tax" money. As such, you have already received "a deduction" by that amount not being included in box 1 of your W-2. Check you year end pay stub for details.
