When you enter your Utah W-2, for the employer's state withholding ID you have to use lower case when you enter the wth (or wmp if it's for mineral withholding). And you have to do do that on all of your forms that have a Utah state employer ID number. All of your Utah W-2's and any Utah 1099's from a Utah payer, etc. that have state withholding on it.
