First, make sure you have not selected any additional states in your 'My Info' section. When you click on Prepare State, if there is a state listed you can use the trash can beside it to delete it.
Check your W-2 to be sure the state name and withholding is blank by using the delete key when in those boxes. Next use the link below to delete a state which should resolve your issue.
