I too ran into this roadblock. I figured out how to delete the form (which was completely blank, except for my name and identifying info) and that solved the problem. I've also helped a coworker who was stuck on the same thing, using the TurboTax online platform. It was a bit more difficult to find the form there, but we did and he was grateful for the help.
TurboTax should fix this problem. If you are not required to use this form, it makes no sense to have it hold up the filing of the state tax return.
