  1. Mass. DOR specifically uses USPS postal mail boxes ("POB") for receipt of all printed/postal submittals. 
    Therefore, it is the fastest and most reliable mode of sending in anything to DOR, except electronic - including payments.
    Using UPS will be slower and less reliable because UPS cannot deliver to a USPS POB but only to, in this case, the Boston Postal Annex location.  
    For fastest submittal and to have a certainty on tracking information and receipt, use USPS Express Mail, and it is usually less expensive than UPS also.  Using Express Mail by USPS you can obtain a certified notice of delivery to the address of record (MASS DOR and its POB)

  2. If your W-2 shows that taxes were withheld (paid), and also substantiates any claim that you paid FICA and Medicare taxes, which are deductible on your MA Form 1, then you would be best to include, paper-clipped, your W-2 to the Form 1.  Technically it could be omitted but would likely slow down the processing.
