Question - You must send in Forms W-2 with your PA tax return.
I lived in MD from January 2021 to end of November 2021 where I moved to PA. I work for the same employer all through 2021 which is located in VA. My W2 came with 3 sections, one for Federal, MD and PA. Box 1 for wages is the same amount for all three. Box 16 for MD and PA are accurate for when I lived in each state. If you add up the sum of Box 16 for MD and PA it equals the total wages of Box 1.
For the PA State Return there is a section titled "Any Special Pennsylvania W-2 Situations: Check all of the following that apply to Forms W-2 you received for 2021."
I assume I should check the following?
- You earned income in another state (I earned income from my employer while I lived in MD)
- Your employer withheld income tax for another state. (Employer withheld tax for MD, during the times I lived there then for PA once I moved.)
- You were a resident of a reciprocal state and your employer withheld PA income tax. (MD is a reciprocal state, and employer withheld PA taxes for the time that I was in PA at end of year.)
- The Medicare wages in box 5 on your Form W-2 are greater than your PA wages in Box 16. (This is true, the medicare wages is almost 24x of Box 16.)
Then the next screen, it states "Prepare W-2 Additional Information Statement: You must send in Forms W-2 with your PA tax return.
Questions:
1. Have I "checked" the proper fields for my PA W-2 Situation?
2. What exact statement/forms I need to provide?
