You’ll need to get that information from the S corporation that you’re a shareholder of. They’re supposed to provide you with a statement with this information.
According to the Instructions for Form CT-34-SH New York S Corporation Shareholders' Information Schedule Tax Year 20...:
The S corporation must provide to each shareholder a statement indicating the shareholder’s pro rata share of the following items:

contributions to one or more of the following New York Charitable Gifts Trust Fund accounts:

Health Charitable Account; or
-
Elementary and Secondary Education Account.

information on the pass-through entity tax (PTET) credit for electing PTET S corporations:

the shareholder’s share of pass-through entity taxable income, and
-
the shareholder’s direct share of PTET credit to be claimed by the shareholder.
