Which State Tax Form(s) to File

I am unsure how to handle my 2021 state tax filing.  

 

In 2021, I lived in OH from 1 Jan to 16 Jul.  Then I lived in KY from 17 Jul through the rest of the year.  

 

I started a full-time job in OH on 22 Jun and have been at that same job ever since.  For the first ~3 weeks at that job, I lived in OH and had OH state tax withheld from my pay.  Upon moving to KY, I filed Ohio Form IT 4NR (Employee's Statement of Residency in a Reciprocity State) with my OH employer, which started KY state tax withholding and stopped the OH state tax withholding.

 

The OH and KY tax reciprocity agreement says if you live in one but work in the other, you'll only need to file a return for the state in which you live.  But in 2021 I lived in both OH and KY, and both were while I worked a job in OH, so what state tax form(s) do I need to file?   Do I file both a KY state tax form and an OH nonresident tax form?  Or just a KY state tax form?  Or something different?
