@DLZ Usually, you do not need to "add" the form. TurboTax (TT) generates it based on your answers to the interview questions.
Explain what you're trying to do. Verify that you're working on an Ohio return. What taxes did your employer improperly withhold (deduct), city, state or school district? Where do your work (city, state)? Where do you live (city, state)? Did you move during the year?
If you have a reciprocal state situation (you are a full year IN, MI, PA, WV, or KY resident) that works in OH and OH state (not city) tax was withheld, there's a simple fix in TT. You cannot get city tax refunded.
