After you file
To mail your return, you will need to separate the federal and state returns.
For your federal return:
- Make sure the return is signed.
- Assemble any schedules and forms behind your Form 1040 and the rest of the forms should be in the order of the "Attachment Sequence No" on the upper right hand corner of the schedule or form.
- Attach a copy of Forms W-2, W-2G, Forms 1099-R if tax was withheld to the front of Form 1040
- If you have a balance, send a check or money order payable to the "United States Treasury."
- Write your name, address, Social Security number, daytime telephone number, and "Year Form 1040"

For your state return:
- Make sure the return is signed.
- Assemble the return in the order that it was printed.
- Attach a copy of Forms W-2, W-2G, Forms 1099-R if tax was withheld to the front of Form 1040
- If you have a balance, send a check or money order payable to your state
