Yes, the amended return is Form 1040-X (two pages). If you have changed other forms, you should include them as well.
For example, you amended to add another W-2. W-2 goes on Form 1040, so there are no other forms to attach.
On the other hand, if you amended to add or change your 1099-B information, you would need to attach Form 8949 and Schedule D because they show your changes.
