After you file

Hi -- I have just about the same problem here, though I'm not using the web (but rather the CD). There are no cookies/cache to clear. Essentially I exited the software and came back a bunch of times (rebooted, etc.), but the same error still shows up where it won't print the file. Now I don't have this 'required paperwork' nor an address on where to send it. Can someone please help?!
