To amend a second time you need
a) a printout of the 1040-X No.1 you sent already,
b) a printout of your truly correct (second iteration) Form 1040 and its attachments.
After that, it's just a matter of filling out the Form 1040-X No.2, where
Col A will show 1040-X No.1 Col C.
