Form 1099-DIV should be an option. Once you've started the amended return and all the totals are zero,
- In TurboTax, select Personal from the top menu.
- On the How do you want to enter your personal income? screen, select I'll choose what I work on.
- On the Your 2021 Income Summary screen, scroll all the way down to Interest and Dividends.
- Select Update next to Dividends on 1099-DIV.
- If you get to the Here's the dividend info we have so far screen, select Add Another Broker or Payer.
- Continue with the onscreen interview until complete.
