After you file

It depends.  You can include the Form 1099-B from Fidelity which will hopefully eliminate any questions later and if you have a Form 8949 be sure to attach that as well to Form 8453.

 

If you would rather not send that now, you should have it printed to stay with your tax return files (actual print or .pdf file stored with your tax return file) so that you can easily access it should you need it later.
