Just an update for the recent commenters. I was ultimately able to get a copy by mail after calling the IRS several times and just repeatedly asking for it until an agent was willing to type all the details into a letter and send to me. They insist they don't do this and can't do this and "there's no such thing" but they absolutely do, can, and it is. Just keep calling over and over until someone is useful.
