They wanted to me fill out that same form I never fill it out I’m giving them till tomorrow if it don’t come today. Due to that form it also have where you have to get it notarized and plus they will take another 30 dollars out your money to reissue the check plus I just didn’t do it because if you can’t even give me where my first check at why would I let you say oh the reissue one is coming on whatever they read off there script.
