Hello, sorry to post on an old thread, But this answer and the form are both too vague. Do you know 'where' to write EIP1 or EIP2 on the top of the form 3911? The top section looks like the IRS is supposed to fill it out, and there is no place to explicitly write in what you are requesting a trace on. Do I just write it on the blank part at the very top of the page? This whole ordeal has been so frustrating and I just want to get this situation resolved but it seems like no one knows.
