I wish I had seen this post before I filed my taxes :( . Unfortunately I never received my second stimulus check and now I have to submit form 3911 and mail it. I live in IL and based on some of the info in this thread I should mail the form to:

Also for stimulus check what should I enter here
"as in reply to your inquiry on ___ about your Federal tax return for ___"
since this is not related to federal tax return? Thanks in advance for the help.
