Once downloaded, you will find it on, Part V of the Federal Information Worksheet.
If you need to find it on the Tax Return you've already filed for this year, go to:
- Tax Home
- Add a State
- Print Center
- Print, Save , or Preview this Year's Return
- Choose Federal
- Download All Forms and Worksheets
