If he has been taking money from an IRA type of account, the document you are seeking is a Form 1099-R. You would need to know the name of the financial institution in order to request a replacement copy of the form. Alternatively, you can request a Wage and Income Transcript from the IRS to get copies of forms that were supplied to the IRS. You can also request a Tax Return Transcript for 2018 to determine if taxes were filed for 2018.
