After 1 week of trying to actavate my green dot turbo card and getting wrong info entered or site did not take my card at all. I was thinking the server was overloaded but I kept trying. The last time I put one month earlier than my birthday and finally the card was approved. If you get a wrong info entered try a month early or a month later for your birthday. Example: if birthday is 04/13/1992 try 05/13/1992 or 06/13/1992. Good luck because you will never find a live person to help.
